Most Common Insurance Plans

We accept all insurance's as long as out-of-network benefits are available. In most cases we are able to do an adjustment to meet in network benefits. This adjustments is made so that the patient is not penalized for using an out-of-network facility. These adjustments are done on a case by case basis. Please call and speak with our insurance verification person before scheduling the case.

CARE CREDIT

We accept CARE CREDIT. CARE CREDIT is a healthcare creditcard specifically designed to pay for treatments and procedures not covered by insurance.  If interested:  Contact 800-365-8295 or go to carecredit.com.

 SELF PAY

You will be contacted prior to your surgery with an estimated procedure cost for your surgery.  The total amount is expected to be paid the day of your surgery. If the total amount is paid in full on the day of your surgery you will receive a 20% discount on the procedure. This discount only applies to the procedure and does not apply to any implants that you might receive. If you are unable to pay the balance in full, 1/2 is expected down and the remaining balance will be due within 90 days from your date of service.  You will be asked to complete a financial agreement.  

SELF PAY – COSMETIC SURGERY  - ELECTIVE SURGERY

Payment in full must be received the day of surgery.
